Home Care

In-home aged care provides support to help you stay independent in your own home for as long as possible. We will assess the most appropriate home care option for you.

Home Care Package:

Assist with My Aged Care to request referrals and apply for a Home Care Package.
Provide a short-list of quality home care providers & guide you to make an informed decision on your selection.
Discuss your care needs during the interim waiting period (12-18 months).
Set up initial meeting your chosen home care provider.
Discuss costs and fees associated with home care.
Future-planning in relation to the need for residential care.

Interim Home Care:

We can apply for appropriate referrals for various services that are available to you in the community. This could be:

Cleaning
Personal Care
Transport
Meal delivery
Social support
Shopping assistance

… and more!

We can access this care whilst you wait for your Home Care Package.

Residential Care

Residential Care is an opportunity for people to live their best life in community-living who require on-going care and support.

We will:

Guide you with future-planning and community living options (advise the difference between retirement/independent living & aged care)
Suggest a selection of suitable residential aged care facilities.
Book tours and coordinate initial meetings with the client services team.
After the tours, assist you in making an informed decision.
Explain all aged care related costs.
Negotiate facility fees to seek a better financial outcome.
Assist you with Services Australia’s income and asset assessment.
Liaise with health and other professionals to coordinate the transition into care.
Provide a 6-week post-placement wellbeing follow-up.
