2. Starting your journey

We will engage with you and your family and listen to your story to better understand your specific aged care needs. (Eg. Desired location, personal, medical, cultural & religious needs and get an insight of your financial situation).

3. Advocacy

Allow us to assist you with the required aged care assessments (My Aged Care), options on home and residential care providers, and general enquiries or concerns from any outside correspondence.

4. Finding suitable aged care

We will then prepare a short list of appropriate home care providers and/or aged care facilities, organise tours and guide you to an informed decision that will suit your current and future needs.

5. Forms & applications

The formal requirements for change of life can be very complex, daunting and time-consuming. Allow us to guide you through the completion of your applications, forms, assessments, and submission to the relevant authorities and organisations, as required.

6. Entering care

We will explain what to expect and guide you along the way. This includes accessing our wide network of resources, services, experts & specialists whom we work alongside and in partnership with.  (Eg. financial planners, medical experts, reliable removalists, estate planning lawyers & private carers.)

7. Follow up

We’re passionate about making our clients feel heard, happy & secure by providing an attentive follow up period. This includes remaining in contact with your family & care provider to make sure the transition has been successful, checking on your loved one’s wellbeing & how they’re settling in, discussion around your contract & finances, and checking on your introduction/referral to other required services.
